Student paperwork

Every enrolled child needs the following before their first day. If anything here is unclear, please just ask. We would far rather answer a question than have you guess.

We’ll email you your family’s private page — it lists every form we need, takes each one as a PDF or a photo, and shows what’s already in. Anything you hand in at the school gets checked off there too.

Immunization exemptions

If your child cannot be vaccinated for medical reasons, you need a Certificate of Medical Exemption signed by a health care provider licensed to give vaccines. That is submitted once, unless your child’s information changes.

Enrolled on or before 10 July 2025: a Certificate of Nonmedical Exemption may be filed for personal or religious belief, at 2, 4, 6, 12 and 18 months — either signed by an immunizing provider, or issued on completing CDPHE’s online education module.

Enrolled after 10 July 2025: children must be fully up to date on MMR. Nonmedical exemptions are still accepted for the remaining vaccinations.

We reserve the right to modify our immunization policy as public health conditions warrant.
